McDonald's (McDonalds) Complaint

I visited your store at walmanr in Clinton MD yeasteday approximately 2p.m. I ordered a Quarter Pounder with cheese and a small Chicken Nuggets off of the "Value Menu"! I received my food in a timely manner and proceeded to walk around Walmart while eating. I took a bite and when i got ready to take another I just happened to look at the bottom side of the burger to discover the breaded was molded! Based on where my first bite was and the location of the mold,I realized that the bite I digested was probably molded also! I went back to McDonald's and showed the sandwich to a cashier. He looked at it and tossed it in the garbage. I asked to speak with the manager. The manager was assisting with food distribution. She was trying to hand me a double cheese burger as if to pacify me while the whole time I was trying to explaing to her what was going on. I'm not sure how fluent she was in english but she either didn't understand what I was saying or she was ignoring me. Either way she didn't seem concerned. Then she said the manager was not present but had "manager" on her nametag! I demanded to speak with her away from the serving line and she finally did reluctantly. I asked for her name and number and asked if there was a regional office i could call to register a formal complaint. Again she acted like a deer in the headlights! I asked for her full name and all she would give me was her first name. I asked for the gentleman's name who I gave the moldy burger to. I told him i was trying to get the managers full name and he was able to assist. I asked the cashier to retrieve the burger so I could take a picture and he complied. What i need to know is what is my and/or your next course of action. Who else do I need to contact? Needless to say, I refused a replacement meal and asked for my money back. The fact is, I ate mold and although it my have be mental, I felt sick to my stomach. Please provide guidence!
